Providence is calling a Behavioral Health Consultant (LCSW).(Behavioral Health, Day Shift, 1.0 FTE, 40 hours per week) to Providence St. Joseph Medical Center in Polson, Montana. Applicants that meet qualifications will receive a text with some additional questions from our ModernHire system!In this position you will: Work's effectively in a patient-centered, integrated model in a variety of clinical settings. Participates in integrated care team meetings to coordinate care and develop plans of care. Determines mental health diagnosis when needed according to current version of Diagnostic and Statistical Manual of Mental Disorders. Construct, document, and execute robust treatment plans using the biopsychosocial model and evidence based interventions. Utilize evidence-based clinical tools for tracking clinical outcomes (PHQ-9, GAD-7, AUDIT-C, etc). Provides brief intervention and/or brief treatment for substance abuse as indicated, adhering to evidence-based techniques. Interface with community substance abuse treatment centers to coordinate care as appropriate (SBIRT). Engages in trainings to learn and improve clinical skills as appropriate. Schedule 30-45 minute appointments for follow-up visits, utilize telephone encounters and MyChart secured messaging as adjunct follow-up to maximize the number of patients followed within IBH. Communicates treatment progress and recommendations to the patient's PCP and other members of the care team. Provide brief behavioral interventions using evidence-based techniques such as behavioral activation, problem solving treatment, motivational interviewing, and other treatments as appropriate. Provide or facilitate in-clinic or outside referrals to evidence-based psychological treatments as clinically indicated. Develop and complete relapse prevention self-management plan with patients who have achieved their treatment goals and are soon to be discharged from the caseload.Required qualifications for this position include: Master's Degree in Social Work Montana Licensed Clinical Social Worker, or license eligible in the State of Montana 3 yearsexperience with adult populations and familiarity with broad range of psychiatric disorders. Highly developed analytical, interpersonal, and counseling skillsPreferred qualification for this position include: Prior experience in integrated behavioral health It is highly preferred that the LCSW achieve his/her OSW-C certification within 5 years of starting this position.
