Job Description:
We seek compassionate Registered Nurse (RN) Case Managers to
provide personalized in-home patient care, developing and
overseeing Plans of Care to enhance patient independence and
satisfaction.
Full-Time, Monday-Friday schedule during normal business hours,
with flexible options.
Compensation is pay per visit with uncapped earnings, processed
bi-weekly.
Benefits include 100% paid premiums for medical, dental, vision,
short-term disability, and life insurance, PTO, mileage
reimbursement, and mobile device allowance.
Primary responsibilities involve patient monitoring, reassessment,
care coordination, education, supervising clinicians, and
participating in discharge planning and case conferences.
Qualifications include 2+ years in acute/post-acute care, active RN
license, CPR certification, valid driver--------s license, auto
insurance, and ability to work collaboratively in a team-oriented
environment.
